There are routes to get support for financial help and support for care. These routes often start with the need for a care assessment. These are carried out by your local authority and, depending on what they advise, will determine what type of support you may be able to get. To get a care assessment the elderly person or the person who can legally make these choices needs to agree to the assessment. Once you have the agreement, you can then request that any current carer, GP or district nurse can refer you on your behalf. Being discharged from hospital can also lead to an assessment as it may be identified that the elderly person needs a certain level of support before the hospital can safely allow the patient to return home.
